Ann Ottesen, Nurse Practitioner
Ann earned her Doctorate in Nursing Practice, Registered in Oregon as an Adult Nurse Practitioner, and specializes in ages 13 and up. She has experience in Urgent Care and Occupational Medicine, is certified thru FMCSA National Registry of Certified Medical Examiners for Commercial Drivers Licenses, has Advanced certification in Wound and Ostomy Care, and has Specialty Certification in Gerontology.

Prior, Ann worked as an RN in Acute Care Hospitals, Home Health and Hospice, Wound Ostomy Care, Skilled Nursing Facilities, Group Homes, Assisted Living, and Memory Care. She is skilled at collaborating with specialists on interdisciplinary teams to manage complex patients and believes that patients and their caregivers belong at the center of the care-team.

Prior to her 20 years in healthcare, Ann earned her first degree in Chinese Literature and Language, learning to read, write and speak Mandarin Chinese. She lived in China both in college and when in the Peace Corps. She loves music and plays the Oboe and Piano. Ann hiked 93 days on the Pacific Crest Trail and lived on a Sailboat in Seattle for 5 years.

Ann lives in Bend, OR with her husband and son, 2 dogs, 30 fish and a Gecko. She loves to ski, run, bike, boat, camp, the performing arts, and culinary endeavors.

Our Priorities
With support from her husband Eric, Ann formed Pacific Sage LLC in 2023. Pacific Sage LLC utilizes existing resources when it makes sense, provides longer appointment times to get to the root of problems, minimize waiting, and promote a relaxed environment, encourages frequent followups when indicated, and strives to maintain a flexible and affordable business model. Our current mission is to provide high quality and accessible primary care for Oregon Health Plan clients in Bend, and to be the top CDL medical exam provider in Central Oregon.
Pacific Sage LLC is Certified with the state of Oregon as an Emerging Small Business (ESB) and Women Business Enterprise (WBE).

Do you bill my insurance company?
We bill your insurance company directly. You will be asked to pay for the portion of your bill that may not be covered by your insurance provider. This may include your deductible. You can contact your insurance company directly to find out more information about what is covered and what is not covered. For Oregon Health Plan clients, all the services should be covered in full.
How do you handle co-payments?
Co-payments are due at the time of your appointment.

Wellness Visits, Established Patient: $200 (must be within 2 years of last visit)
